Right Clinician, Right Time, Right Place

In addition to our GP / Nurse Practitioner / Emergency Care Practitioner the surgery can offer you: –

• Social Prescriber – Able to assist with social, economic and environmental issues
• Physiotherapy – Pure Physio hold face to face clinics at the surgery
• Mental Health Practitioner – The surgery have 2 practitioners that are available to see people to assist.
• Health and Wellbeing Coach – Able to do some Health Care Assistant work and also is able to assist patients with wellbeing with Long Term Conditions and speak to patients who need support.
• Care Co-Ordinator – Able to help patients by doing some Health Care Assistant work and any patients that needs social support.

To make an appointment with any of the above, except Mental Health Practitioner – GP recommendation only, speak to our Reception team or complete a ‘I want help with a medical condition’ form and this will be assessed and appointment or telephone call organised for you.
