You can download the NHS app or log on your computer https://www.nhs.uk/nhs-app/ and this will authorise you for online services to order your medication.

You can complete the form on the website ( click on 'Online Services' above) and attach your 2 forms of ID to this for an application for online services to the surgery and we will email you back with log in details.

You do not need to come into the surgery unless you wish to be a proxy for a member of the family or close friend and you will need to complete a form with the patient. Thank you.
